NUCCAFORMS.COM is a PBR (Practice-Based Reasearch) project in which new patients in a chiropractic clinic will fill the clinic's initial questionnaire directly on the internet. The chiropractor will receive the completed questionnaire in e-mail format in his e-mail box.
